To help save time during your first visit to Rheumatology Specialists of Connecticut, please fill out the following forms. The Patient Registration Form helps to provide the practice with some basic information to help begin your treatment. Also, upon your FIRST VISIT to Dr. Quan’s office, we will require some basic medical history to help shape the course of treatment. In order to do so, please fill out our New Patient Intake and Medication List forms.
If your visit is for Osteoporosis, please fill out our Osteoporosis form.
If you are ALREADY a patient of Dr. Quan’s, please help us keep track of any changes that may occur between visits. In order to do so, please fill out our Follow-up Intake form.
If your visit is for Rheumatoid Arthritis, EVERY VISIT will require you to fill out our quick and easy HAQ Assessment form, which will help us monitor your disease activity.
We may need access to medical records held at other medical facilities or by healthcare providers you have seen in the past. In order to do so, please fill out and sign the medical release form below:

Will Dr. Quan take my insurance?
Dr. Quan will be able to take most commercial and Medicare insurances. Unfortunately, we are unable to accept Wellcare, HealthyCT, Colonial, Humana, Evercare, Secure Horizon, Medicaid, Husky, QMB, any Dual Complete, or any Health Exchange insurances, including CtCare Exchange and the Access Health CT bronze plan.
